After your deposit has been processed, you will be returned to a registration form. Complete the information and click the submit button to send it to Beth Bradley Dog Training.

THIS IS A NON-REFUNDABLE $75 DEPOSIT.  SHOULD YOU BE UNABLE TO ATTEND THE CLASS, YOUR DEPOSIT WILL NOT BE REFUNDED.  YOUR DEPOSIT MAY BE USED FOR ANOTHER CLASS OR PRIVATE LESSON.  THE BALANCE OF THE CLASS IS DUE AT THE FIRST LESSON.

ALL DOG’S MUST BE CURRENT ON VACCINATIONS.  ALL DOG’S MUST BE ON A FLEA & TICK PREVENTATIVE.

ALL DOG’S MUST BE FRIENDLY WITH PEOPLE AND OTHER DOGS.

ALL CHILDREN MUST BE ACCOMPANIED BY AN ADULT.  NO CHILD WILL BE PERMITTED TO RUN OR DISRUPT THE OTHER DOGS IN THE CLASS.  ONE PERSON IS TO HANDLE THE DOG AT A TIME.
